By participating in the Hotlanta Strikers Tournament, I understand that:
I know that playing sports can sometimes result in injuries, and I'm participating by choice.
I'm responsible for my own health and any medical care I might need during the tournament.
I have health insurance or accept responsibility for any medical costs if I get injured.
I won't hold the Hotlanta Strikers Club, organizers, or venues responsible if I get hurt.
Important: Following tournament rules and playing safely is required. Unsafe behavior may result in removal from the event.
Once you complete this form, submit it to the tournament committee before your first game. That's it - you're all set to play!
